Qualify to be a Supplier

Congratulations on being selected to supply consultants to Intel. Intel manages its staffing suppliers through Yurcor's Vendor Management service. You will deliver services to Intel as a Yurcor subcontractor. Intel requires paperwork to be completed promptly so your immediate attention is needed. Complete both steps before you start your first consultant:

1.	Download these Supplier Qualification Forms and fax them back to Yurcor at 561.278.1568. If you wish to scan the documents, email them to support@clearpathpay.com. 2.	Have your insurance provider fax and mail an original Certificate of Insurance to Yurcor.

Insurance Certificate Requirements

• Commercial General Liability and Automobile Liability Insurance with limits of liability not less than $1,000,000 per occurrence and including liability coverage for bodily injury or property damage (1) assumed in a contract or agreement pertaining to Supplier's business and (2) arising out of Supplier's product, Services or work. Supplier's insurance shall be primary, and any applicable insurance maintained by Buyer or Buyer's Client shall be excess and non-contributing. The above coverage shall name "The Churchill Benefit Corporation dba Yurcor" as additional insured.
• Statutory Workers' Compensation coverage including a Broad Form All States Endorsement in the amount required by law, and Employers' Liability Insurance in the amount of $1,000,000 per occurrence. Such insurance shall include an insurer's waiver of subrogation in favor of Buyer.
• If Supplier is providing any professional service to Buyer's Client, Supplier shall maintain Professional Liability Insurance (including errors and omissions coverage) with liability limits not less than $1,000,000.
